I wonder if your question reflects a way of relating that is peculiar to your generation, which I am guessing is much less formal than mine? In all of my relationships, I was taught to always address a person I did not know, or someone older than myself or in a higher position by his or her surname. It was (and still is, in my view) a breach of etiquette and a mark of familiarity to call someone with whom you have no relationship by his or her first name. I believe in professional correspondence it is safer to follow that rule than to risk offending someone by using first names. I am interested in hearing the opinion of my colleagues, who may be more "with it" in the ways of doing things these days than I am ;-)
