| Appeal letter salutations

I am hoping that some folks can offer me insight into appeal letter salutations. Is there a rule of thumb regarding addressing someone by their first name vs last name? Does it just come down to formal vs casual? Or, does one salutation typically illicit a greater response rate? In my own mailbox, I find that I respond more favorably to letters that refer to me by first name. Any thoughts are appreciated.
