In Pennsylvania, where I live and work, our contracts must include these two sentences: the consultant (name) "will not at any time ask for a contribution, nor will she have custody or control of contributions. The (non-profit) has the control and approval over the content and volume of solicitations."
The other sentence I always add is" "this contract can be terminated by either party with a two-week notice."
I do not think it is necessary for you to consult an attorney.
In terms of how many hours you will work, I think the way you have explained it sounds good. I would commit to ten hours a week until you get a feel for what has to be done and then increase, but ask the client to commit to this first.
