Ronda:Have you searched the Dept. of Health & Human Services? You are serving low income, rural, and if you qualify for "at-risk" as well, you may be eligible for various programs at first glance. The other are USDA programs, in terms of Rural Development grants.
I would also contact your local state and federal representatives for help. They often get funding directed to rural development...clinics are a HUGE plus for them in terms of Public Relations during campaign season.
There are several foundations which support health care initiatives for children/youth around the country. If you have access to a database search like the Foundation Center offers, I believe you will find some good matches of interest.
