nursing student learning about crohn's, would appreciate input!

As part of a chronic illness class I have, I had to prepare a questionnaire and find at least two people that have Crohn's to answer the questionnaire. Could you guys help me out? I'm really just trying to learn about the disease and how to best manage it from a nursing perspective. I pasted the questions in the bottom of this thread. Maybe you could answer the questions and email them to me? No need for full name and contact info or anything, this can be as anonymous as you'd like it to be.
When did you first begin experiencing symptoms?
Have your symptoms been continuous or intermittent?
How severe are your symptoms?
How often do you experience flare-ups?
Have you officially been diagnosed with Crohn’s disease or ulcerative colitis?
Do your symptoms affect your ability to work or do other activities?
Does anything seem to improve your symptoms?
Is there anything that you've noticed that makes your symptoms worse?
Do you smoke?
Do you take no steroidal anti-inflammatory medications (ibuprofen, naproxen, diclofenac, others)?
What is your current treatment regimen?
How do you feel your current management plan is working?
What changes would you like to see in your current management plan?
Have you made any diet changes after being diagnosed? What changes did you make? How is this working?
Have you utilized any support groups for IBS? How have they helped? Would you recommend them to others?
Are you currently taking any medications to manage your IBS? What side effects have you experienced with that medication? Do you think it works well for you?
What questions do you still have about IBS? What resources do you turn to to gather information about your illness?
What is your occupation? How do you think your job affects your IBS?
Have you considered alternative therapies? Do you feel like they’ve worked well for you?
How has IBS affected your family life?
